New Video Series Supports Teens Transitioning from Pediatric to Adult Care
Transitioning from pediatric to adult health care is a major milestone for young people. To help make this transition smoother, The Cooper Foundation in partnership with the Esther & Pedro Rosenblatt Foundation has launched a new educational video series featuring experts from Cooper University Health Care.
The series was made possible through a $50,000 grant from the Esther & Pedro Rosenblatt Foundation, the videos are designed to provide practical guidance for teens, young adults, families, and caregivers as they navigate the shift from pediatric to adult care.

The series features Cooper clinicians sharing practical advice on topics that play a key role in maintaining health and independence during this transition.
Nutrition and Building Healthy Habits
Registered Dietitian Anne Star, RD, discusses how thoughtful nutrition can help young adults with chronic conditions stay healthy and confident as they begin managing their own care.
Finding the Right Adult Provider
Advanced Practice Nurse Marilyn Clifford, APN, explains the importance of establishing care with a new adult provider and taking ownership of scheduling appointments, managing medications, and building relationships with a new care team.
Mental Health and Managing Stress
Anthony Rostain, MD, MA, Chief and Chair of Psychiatry, addresses the emotional challenges that can come with increased independence. He shares strategies for managing stress, including limiting screen time, exercising, spending time outdoors, and maintaining strong social connections.
